QualChoice of Cleveland is no longer in the health care market. As of December 31, 2006, all health plans have been terminated.

QualChoice will continue process the following:

• Eligible claims for commercial members for up to one year from the date services were provided
• Eligible claims for Medicare members for services obtained within the timely filing requirements
• Appeals of denied claims filed within 60 days of the date on the EOP
