Heritage Value Plus 30™
Provides coverage for accidents and illnesses, with a choice
of deductibles. When
using a health care provider from the Premera network, you
will save in out-of-pocket costs. This is a low annual premium
plan.
Heritage Preferred Plus 30™
Delivers major medical, routine, and preventive services with
a single annual deductible. There is coinsurance for
Premera network providers.
Heritage Preferred Plus 20™
Provides preventive, routine, and major medical services with
a single yearly deductible. There is a modest coinsurance for Premera network
health professionals.
Long Term Care Insurance
Furnishes
freedom to choose the type of care you want. In the Comprehensive
Plan, you are not limited to one type of service, so you can
get the help you need at home or at a facility. In the Facility
Only plan, benefits come when the member receives care at
a qualified facility. In the Community Only plan, benefits
come when the member is receiving home care and not at a facility.
