PacifiCare Signature Value HMO

An emphasis on in-network benefits make this option an affordable and reliable solution for individuals and families. Your health is managed under the guidance and referral of your PCP, or Primary Care Physician. Costs are low and predictable, and paperwork is minimal.

PacifiCare Signature POS

The PacifiCare Point of Service product offers an increase in coverage flexibility, partnered with the savings of an HMO. Keep out-of-pocket costs low by choosing to visit an in-network provider.

PacifiCare SignatureOptions PPO

This PPO option gives you access to an even larger network of providers, so that you have optimum control over your healthcare. PPO members also have the freedom to seek out-of-network care, but at a higher out-of-pocket cost.

PacifiCare Signature Independence Indemnity

Signature Independence is a standard indemnity plan, providing members with the freedom to see any doctor without a referral. Plan details include an annual deductible, and out-of-pocket coinsurance payment for all covered services.

PacifiCare Signature Freedom SDHP

This Self Directed Health Plan provides members with the freedom of a PPO and an annual Self Directed Account (SDA) to allocate funds as per member preferences. Members can choose any licensed provider for treatment, and any SDA funds not spent can be carried over to the next year, in effect acting as a 'savings account' for future health expenses.
