PPO Plans from LifeWise

Offer a statewide network of 6000 physicians and health care providers whose services will provide you with higher benefit levels and lower out-of-pocket expenses. You do not need referrals for services and there are flexible plan designs available including:

Traditional indemnity plans which provide you with freedom of choice for health care providers. There are several choices for deductibles, benefit levels, and coinsurance which allow you to tailor the plan to your needs.

HSA Choice Plans

These include traditional indemnity plans with a qualified high deductible and lower annual premium, combined with a Health Savings Account. You open an HSA and deposit tax advantaged money to pay for the annual deductible and other allowable medical expenses. Money unused and left in the account can gain tax-free interest or be used later for medical needs, giving you more control of your money.
