IBC Individual Health Plans
Personal Choice® PPO Plan
Offers
the services of the Personal Choice network of health care
providers while also giving you the freedom to seek care from
outside the network. This plan includes wellness programs,
chronic conditions management, and discounts on alternative
services. Use of the Personal Choice network offers the highest
level of benefits after co-payments. There are choices to
tailor the plan to your personal needs and budget.
Traditional Indemnity Plan
Offers comprehensive medical, surgical, and hospital coverage
with an optional major medical supplement. This traditional
plan allows you to seek medical attention from any healthcare
provider of your choice and provides benefits on a fee-for-service
basis. This plan is available in certain counties of PA only.
Student Health Insurance
Offered to college students under age 26 who want comprehensive
coverage at a lower cost. This plan is only offered to residents
and students of Bucks, Chester, Delaware, Montgomery, or Philadelphia
counties. This plan may also be available to students whose residence is in the five-county Philadelphia area attending post-secondary school elsewhere.
Sole Proprietor Health Insurance
Available through associations, such as a Chamber of Commerce,
which can acquire group health coverage through Keystone Health
Plan East HMO. It is intended for the self-employed who do
not have any employees.
Personal Choice 65
A Medicare
Advantage plan† which provides a Medicare contract through
the PPO (Preferred Provider Organization) network of Independence
Blue Cross. You have Medicare benefits with the added services
of preventive care, prescription drug coverage and more. Your
benefits are greater when seeking care from within the PPO
network of health providers. You are free to seek medical
care from out-of-network, but those visits can be more costly.
Keystone 65
A Medicare Advantage
plan which provides a Medicare contract through a HMO (Health
Maintenance Organization) of Independence Blue Cross. You
choose a Primary Care Physician from the HMO network and agree
to stay within the network for other services. There are no
annual deductibles and small co-payments. Keystone 65 Choice
– Point-of-Service (POS) Option is available which allows
you access to both in-network and out-of-network physicians
without a referral. Plans are available in only some counties.
Security 65
A Medicare supplemental
plan which helps cover expenses that Medicare does not. There
is a choice of four options to choose from, so you will be
able to refine your plan according to your needs.
Low Income plans
Include Special
CareSM, CHIP, adultBasicSM, and Keystone Mercy Health Plan.
