Health Net Coverage and Plans
Individual and Family PPO
With the Health Net PPO product, your family can choose between Preferred Provider Organization (PPO) and Out-of-Network benefit levels on an incident basis. See any provider in the Health Net network without a referral, or go out-of-network with an increase in out-of-pocket costs.
Individual and Family HMO
The Health Net HMO option offers members outstanding coverage at a lower cost than the PPO option. Members choose a Primary Care Physician (PCP) from Health Net's network, and through their relationship with that provider all referrals and claims are formerly processed. There are no variable expenses, and out-of-pocket costs remain low.
