accessibility of doctors, and coinsurance (costs you shoulder

coverage you are looking for.

Fallon Community Health will help you decide
which plan is best for you and help you structure it to suit
your unique needs. For residents of Massachusetts, FCHP offers
the following plans.
Fallon CHP Individual Health Plans
FCHP Direct Care
Provides HMO
(Health Maintenance Organization) care with several options
for plan design. There are no co-payments for routine care
with your personal care physician or gynecologist. Most of
your care is through the Direct Care network of health care
providers. Plan includes extra features.
FCHP Select Care
Features an
HMO and the added availability of the large Select Care network
of health care providers in your region of service. When you
choose your primary care physician from the Fallon Clinic,
you may self-refer yourself to any specialist within the Clinic
also. There are several options for plan design. Extra features
are included with this plan.
FCHP Flex Care
Provides a POS
(Point of Service) plan with access to the Direct Care network
associated with the HMO. You also have the choice to seek
care from outside the network but at a greater out-of-pocket
cost. You have extra features with this plan, plus bonus benefits.
FCHP Select Care
Provides a
POS (Point of Service) plan with access to the broader Select
Care network associated with the HMO. You can choose to see
doctors outside of the network, but it will cost you more
out-of-pocket. Extra features are included, as well as bonus
benefits.
Fallon Senior Plans
These are Medicare
Advantage health plans.† Each provides your Medicare coverage,
as well as the additional benefits associated with each plan.
Your care is obtained through the FCHP networks of health
care providers. With some plans, you may seek care outside
of the networks. The plans include: Senior Saver Plan, Senior
Plan Standard, Senior Plan Plus, and Senior Plan Preferred.
Fallon Preferred Care PPO
Assures
your healthcare through a country-wide network of health providers
or the Fallon Clinic. Care you receive through the network
professionals will provide you with the highest benefits.
The network includes over 15,000 providers in Massachusetts
in addition to the Fallon Clinic providers. You also have
out-of-network coverage.
Major Medical
Offers traditional
indemnity health insurance. Here, you can seek care from any
physician you want. After paying your annual deductible, insurance
will pay 80% coinsurance for covered services. Plan includes
extra features.
FCHP Independent Care
A guaranteed issue health plan, where you are accepted regardless
of your medical history. You have a choice of health provider
networks, hospital services, prescription drug coverage, and
wellness features.
FCHP Independent Care 1000
Adds an out-of-pocket deductible and does not include prescription
drug coverage. This allows the plan to be offered with a lower
premium.
FCHP MassHealth and Elder Service Plan
Both offered by Fallon Community Health Plan to low-income
qualifying residents of Massachusetts.
