Excellus BC/BS of New York Individual Health Plans
Blue Choice
Offers an HMO, Health Maintenance Organization, network of quality health providers and offers two plan options: the original Blue Choice Personal and the more flexible Blue Choice Personal Point of Service (POS). In the Blue Choice Personal HMO, you choose a primary care physician who oversees all of your medical needs and supplies referrals to specialists accordingly. The Blue Choice Personal Point of Service works the same way, but you do not need referrals to seek care from out-of-network providers. This allows you more freedom, but at higher costs.
Blue Choice Senior
Offered to those who are eligible for Medicare.† It provides a participating network of health care providers for comprehensive coverage and low co-payments for routine and covered services.
Medicare Supplemental
Offered to those enrolled with Medicare Parts A and B and who are looking for additional benefits that coordinate with Medicare. There are seven levels of supplemental insurance from which to choose in order to target your level of need.
Medicare Blue Choice
Offers comprehensive coverage, over and beyond your Medicare, for those living in the seven New York counties. There are three plans within Medicare Blue Choice from which to choose: Optimum, Value, and Platinum.
Traditional Indemnity
A comprehensive health plan and offers total freedom of choice to seek care from any physician you want. When seeking care from within the Excellus network of providers, there is no need for claim forms.
Excellus BlueCross BlueShield of NY Low Income Health Plans
Blue Choice Option
An HMO/Health Maintenance Organization network of 9000 quality health providers for those with Medicaid. You choose a primary care physician who will manage all of your health care needs.
Child Health Plus
Administered by Excellus BCBS for infants to children of 19 years of age as a free or low cost comprehensive insurance plan.
Family Health Plus
An HMO for those 20 to 64 years old, who have incomes high enough to make them ineligible for Medicaid, but who could not afford health insurance otherwise.
Healthy New York
This is group insurance through a NY-sponsored program.
ValuMed
Comprehensive health insurance for those who qualify. It is offered as low cost health insurance for those who have been recently unemployed or who have low incomes and live in certain counties of New York.
ValuMed Plus
Comprehensive health insurance for those who qualify. It is offered as a low cost insurance plan to those whose jobs do not offer health benefits and who are not eligible for government programs. Only residents of certain counties of New York qualify.
