Empire BC/BS of NY Individual Health Plans
Direct Pay HMO/POS
Combines the advantages of a Health Maintenance Organization network of health care professionals with the additional freedom of a Point Of Service to seek care out-of-network. You choose a primary care physician from the HMO network and he/she coordinates your medical care. Many services from the HMO network are no cost or require a small co-pay, with additional costs for services outside the network.
Direct Pay HMO
Provides the network benefits of the Health Maintenance Organization from which you choose your primary care physician. He/she manages your medical care through the HMO network of health professionals which is your only source of covered services.
TraditionPLUS Hospital Plan
Offers paid-in-full hospital benefits at over participating hospitals in the tri-state area covered by Empire BCBS of NY. You also have access to the nationwide network of hospitals contracting with Blue Cross Blue Shield.
MediBlueSM HMO
Provides an HMO to members, with no referral needed to see any other network specialists. You still choose a Primary Care Physician from the HMO network to supervise your medical needs, but you have greater freedom – no need for the referral usually required from a PCP for other doctors. This is a Medicare Advantage plan, authorized by the government, which you may choose instead of traditional Medicare.
Direct Payment Medicare Supplement
Offers three types of supplemental plans from which to choose and provides extra benefits and covered costs not always paid by Medicare.
Child Health Plus and Healthy NY
Both of these plans are provided by Empire BCBSNY for low income families.
Empire HealthChoice Assurance, Inc. and Empire HealthChoice HMO, Inc. provide insurance services through the BCBS Association.
