Carefirst BCBS of MD Individual Health Plans
BluePreferred PPO
This option allows you to coordinate your coverage to meet your own needs and budget. This includes the freedom to select any doctor, specialist or hospital you wish, saving on out-of-pocket costs by selecting one of the providers within the PPO. You also have freedom to go out-of-network, but at slightly higher costs. Other benefits include preventative care, hospital and physician services, prescription drug benefits, and the option to extend maternity coverage.
BlueChoice HMO
This referral-based coverage option offers optimum coverage and protection, within a network of primary care physicians (PCPs). There are various co-payment options for office visits, no deductibles, preventive care, such as, well-child care, immunizations, cancer screenings, and vision exams. You can select a Primary Care Physician to coordinate your care. You will also receive emergency care at just one co-payment, prescription drug benefits, and optional dental benefits as well.
BlueChoice Opt-Out POS
CareFirst's
POS option offers the coverage and security of an HMO with
freedom from the PCP referral requirements that typically
characterize an HMO. Members pay a lower out-of-cost fee for
in-network physicians, but are not limited in treatment allowance.

and Open Access variations on their standard offerings.
