Blue Cross Freedom Blue PPO I and II

As of 2009, there are two different deductible levels, depending on the plan. The monthly premium (in addition to Medicare Part B premium) is $0 for Freedom Blue I and $50 for Freedom Blue II. Includes in and out-of-network coverage. In-network doctor and specialist visits are only $10.

Low premiums; low co-payments; vision, dental, podiatry coverage; available in only some CA counties.

Private Fee For Service Plan: monthly premiums of from $15-50; low co-payments; no deductibles; only some counties.
