Wisconsin, including the Personal Choice Plans, will allow
you to pinpoint the exact coverage you want at a cost you

BC/BS of WI Individual Health Plans
Comprehensive Major Medical Plans
Offer the PPO/Preferred Provider Organization network of health
care providers, at a cost savings, with the choice to seek
care out-of-network. You have a choice from six levels of
deductibles that come with two levels of coinsurance benefits.
Value Plan PPO
Provides comprehensive
coverage at a lower premium. Your use of the PPO network gives
you additional out-of-pocket savings. You have a choice of
five deductibles and lower coinsurance for covered services.
CompcareBlue™ HMO Plan
Delivers the Health Maintenance Organization managed care
method of health care from BCBXWI. You choose a Primary Care
Physician from the extensive BCBS network of HMO providers
and he/she coordinates all of your medical care. You have
a choice of three deductibles; after you reach your annual
deductible, the plan will pay coinsurance for all of your
network services. This plan is available in 10 Wisconsin counties.
Health Savings Account (HSA) Plans
Offer
a tax advantaged way to pay for health insurance. These plans
couple a high deductible comprehensive PPO insurance plan
with a Health Savings Account. The PPO plans with a high deductible
have lower premiums. The HSA uses the tax exempt money you
deposit there for your insurance deductible and other allowed
services; money not used is allowed to remain in the HSA and
accumulate tax free. You end up saving in several ways.
Hospital Surgical Plans
Provide
for possible illnesses or accidents that would involve hospital
stays or surgery. There are six deductibles from which to
choose. You have freedom to seek care from any medical provider,
but use of the BCBS network will provide a greater savings
for you.
TempPlan
Delivers health insurance
to those who only need it for a short time. There is a choice
from four deductible levels, coinsurance levels, and a time
periods from 30 to 180 days.
LifestyleBlue
A discount
program designed to promote health and wellness. Members derive
discounts on a wide range of services and products that can
lead to better health. Services involve vision, hearing, home
health equipment, spa services, childcare, and much more.
Health Plans for Seniors
Offer
several two Medicare supplemental plans: Value Plus and SmartChoice
High Deductible. The Medicare Advantage Plan is the SmartValue
Private Fee For Service plan.
