Horizon BC/BS of NJ Individual Health Plans
Horizon Basic Plan
Offers the Horizon Traditional Network of health professionals for your out-of-pocket savings, while also giving you the scope to go out of network. You have a choice of several annual deductible levels while paying 50% coinsurance for covered expenses.
Horizon Traditional Plan B
A fee-for-service plan where you again have the freedom to choose any health professional services, but save by seeing those with the Horizon Traditional Network. You have a choice of several annual deductible levels while paying 40% coinsurance.
Horizon Traditional Plan C
A fee-for-service plan where you again have the freedom to choose any health professional services, but save by using the Horizon Traditional Network. You have two choices for annual deductible and only pay 30% coinsurance.
Horizon Traditional Plan D
A fee-for-service plan where you again save by using the Horizon Traditional Network but have the freedom to choose any health professional you want. There are two choices for the annual deductible and you will only pay 20% coinsurance for covered expenses.
Horizon MSA Companion/High Deductible Plans C & D
Provide health coverage with either of two options (Plan C or Plan D) and works in combination with a Medical Savings Account, a tax advantaged means of paying health costs. You must determine whether your situation works with an MSA. There are four deductibles from which to choose in either plan and both are available without the MSA. With the option of Plan C, you pay 30% coinsurance and 40% with Plan D.
Horizon HMO is a Health Management Organization
Where all of your health care needs are managed by your choice of a primary care physician (PCP). There is an extensive choice of thousands of physicians from which to choose. The HMO offers low out-of-pocket expenses, while providing complete comprehensive care. There is no annual deductible, no coinsurance, and low copayments for office visits. Wellness and prevention programs are emphasized.

Horizon Individual Dental
Offers coverage for certain dental and preventive procedures when seeking care from the Horizon Dental Choice Network of dentists. There are no deductibles or copayments required.
Horizon Centurion Dental
Offers coverage for eligible dental procedures when seeking care from the Horizon Dental Choice Network of dentists. Here, you pay a reduced fee for covered services, according to an agreement between your dentist and Horizon.
Dental plans are through Horizon Healthcare Dental, Inc.
