Provides

any physician or hospital in the country. It is underwritten
by World Insurance Company and offered solely through American
Health Underwriters. There are several options available within
this plan and exact benefits may vary according to your state
of residence. This plan is offered in the following states:
Alabama, Arkansas, Delaware, Florida, Georgia, Illinois, Indiana, Minnesota,
Missouri, Michigan, Nebraska, New Mexico, North Carolina,
Ohio, Oklahoma, Pennsylvania, Tennessee, Texas, South Carolina, Virginia,
Wisconsin, and West Virginia.
Small Business Association Plan
Goes beyond regular coverage and provides the following added
benefits for members of the Small Business Association of
America: Accidental Injury Excess Coverage, Dental Plan, Vision
Plan, International Medical Air Transportation Service, and
Chiropractic Discount Plan.
Savers Advantage Plan
Provides
each individual with the opportunity to choose one’s
own doctor or hospital. It is an indemnity policy for basic
hospital, medical, and surgical expenses with several options
of services from which to choose. This freedom of choice plan
is underwritten by Southwest Service Life Insurance Company,
