ABA National Health Card
Delivers
up to $20 discounts with network physicians and specialists,
up to 20% discounts with participating hospitals, up to 20%
discounts with network medical ancillary providers, the triple
prescription discount card, up to 20% discount with participating
dentists, 20% to 65% discounts on eye care purchases at network
optical outlets, up to 20% discounts with network home health
care and durable medical equipment providers.
U. S. Health Card
Provides
up to 20% to 35% discounts at network physicians, up to 20%
discounts with participating hospitals, up to 20% discounts
with ancillary services, a national prescription card, Canadian
Prescription Connections, up to 20% discount with network
dentists, from 20% to 35% discounts with participating optical
outlets, up to 20% discounts with home health care and durable
medical equipment providers.
Triple Prescription Discount Plan
Delivers access to the 50% to 80% discounts at Canadian pharmacies,
30% to 40% discounts with U.S. mail orders, an advance PCS
card with up to 20% local pharmacy discounts.
Millennium Plan
Offers discounts
of up to 20% at network physicians, up to 25% discounts on
additional medical needs, up to 20% discounts at participating
hospitals, up to 20% discounts at participating ancillary
facilities, a prescription card, up to $2000 accident coverage,
up to $2000 accidental death or dismemberment coverage, a
dental discount program, up to 60% discounts at participating
vision centers, and a wide range of consumer discount benefits.
ABA Dental Program
Delivers
substantial savings with participating dentists on multiple
types of dental procedures without claim forms, maximum benefits,
deductibles or waiting periods.
All-American Alliance Benefits
Offer minimum of $40 rebates on all types of medical and dental
providers, up to 20% on excluded insurance procedures, additional
discounts on additional medical provisions like home health
and equipment, up to 40% discounts at participating hospitals,
up to 50% at ancillary facilities, the Triple Prescription
Discount Plan, $2000 accident coverage, $2000 accidental death
and dismemberment coverage, both dental and vision programs,
and a wide ranging consumer discount plan.
ABA Ultra Benefit Plan
Provides
an average 35% discount with your network physician and 20%
with participating hospitals, accident disability income of
up to $250 per week, discounts on additional medical needs,
accidental death or dismemberment coverage, triple prescription
discounts, dental and vision programs, and a wide range of
consumer discounts.
Accident Disability Income Plan
Includes the ABA National Health Card, has accident disability
income of $250 per week for 52 weeks (with 14 day elimination
period), accidental death or dismemberment of $10,000, and
all members of the family are eligible.
Critical Care Program
Provides
additional benefits of $2500 to $10,000 in the event of a
stroke, heart attack, coma, kidney failure, life threatening
cancer, coronary artery bypass, major organ transplant, loss
of sight, hearing or speech, paralysis, or severe burn. Also
includes the ABA National Health Card.
Business Benefits Management
Offers discount programs for prescriptions, dental, hearing
and vision, optional family accident plan, optional accident
disability income plan, easy employer payment terms, and availability
of an ABA Business Services Representative.
