Types of PPO health plans in Arizona, California, Connecticut, Georgia, Illinois, Ohio, and Texas

  • High Deductible PPO1 Plan is compatible with Health Savings Accounts and allows a customer to access the Aetna PPO network of doctors and specialists. The deductible is high in this plan, then pays 20% of covered services and has low premiums.

  • High Deductible PPO2 Plan is compatible with Health Savings Accounts, allowing an individual to access the Aetna PPO network of doctors and specialists. After fulfilling the high deductible, a policy holder has no charges for covered medical visits and services and will pay a low annual premium.

  • PPO 500 Plan has the lowest deductibles, lowest co-pays for office visits, lowest maximum out-of-pocket payments, and the highest premiums.

  • PPO 1500 Plan has low deductibles, low co-pays at doctor visits, low total out-of-pocket expenses, and a premium that is moderately high.

  • PPO 2500 Plan features a moderate premium, with deductible, co-pay, a maximum annual out-of-pocket expense, and service coverages that are also in the moderate range.

  • PPO 5000 Plan has a lower premium, with a higher deductible, co-pay, out-of-pocket maximum, and service coverages.

California residents have the following additional choices for PPO health plans

  • PPO Value 1500 Plan has a lower deductible with lower maximum out-of-pocket expense and a higher premium.

  • PO Value 2500 Plan has a higher deductible, co-pay, and out-of-pocket maximum which allows for a lower premium.

  • PPO Value 5000 Plan has a low premium with a high deductible, co-pay and yearly out-of-pocket maximum.

Georgia residents have an additional insurance plan

  • PPO Value Plan features low office visit and pharmacy co-pays with low maximum out-of-pocket yearly expenses, requiring a moderate deductible and annual premium.

Types of PPO plans in Delaware, Maryland, Pennsylvania, Virginia, and Washington, D.C.
