Individual Choice Basic and Essential
Health Care Services Indemnity Plan
Features a
low deductible with wellness benefits for its customers. There
are low out-of-pocket annual maximums for covered services.
The annual premiums are high, with different rates in different
areas of the state.
Individual Choice A-D Indemnity Plans:

Plan A/50: moderate deductible; highest
out-of-pocket maximum; preventive care; 50% coinsurance;
lowest premium.
